홍콩 공공의료 체계가 대전환점을 맞이했다. 홍콩 병원관리 당국은 지속 가능한 의료 재정과 응급실 남용 방지를 목표로 공공병원 요금 체계를 대대적으로 개편했다.
개편의 핵심은 단순한 가격 인상이 아니다. 진정한 응급 환자에게 자원을 집중하되, 경제적 약자에게는 문턱을 더욱 낮추는 '선별적 공공성'의 강화이다.
기존 단일 가격 체계에서 응급도별 차등 요금제로 전환되었다.
응급실(A&E) 기본 진료비: 기존 180 HKD에서 400 HKD로 상향 조정
중증 환자 전액 면제: 트리아지(Triage) 분류 기준 Category 1(위독, Critical) 및 Category 2(응급, Emergency) 환자는 진료비 400 HKD가 전액 면제되어 무료 치료를 받는다.
일반 외래 및 검사비: 일반 가정의학 외래 처치·주사료가 19 HKD에서 50 HKD로 조정되었다.
정밀 영상(CT·MRI) 및 병리 검사 시 50~500 HKD 수준의 코페이먼트(Co-payment, 일부 본인 부담) 제도가 도입되었다.
연간 본인 부담금 상한제: 환자의 의료비 부담 폭증을 막기 위해 연간 본인 부담금 상한선 10,000 HKD 제도가 신설되었다.
저소득층 감면 대상 확대
진료비 인상에 따른 저소득층의 의료 소외를 막기 위해, 홍콩 정부는 수혜 기준(Means Test)을 대폭 완화했다.
1인 가구는 중위 소득의 150% 이하, 2인 이상 가구는 중위 소득 100% 이하로 자격 요건을 완화하고 자산 기준도 공공주택 신청 자격 수준까지 올려 적용 대상을 넓혔다. 그 결과, 과거 약 1만 4천 명 수준에 불과했던 상시 의료비 감면 혜택 대상자가 140만 명 이상으로 대폭 확대되며 취약계층의 실질적 의료 접근성을 크게 끌어올렸다는 평가를 받는다.
의료비 감면(Medical Fee Waiver) 신청 가이드
감면 혜택을 받기 위해서는 자격 요건 확인 및 서류 심사 절차가 필요하다.
신청 채널: 각 공공병원 내 의료사회복지실(MSSU) 방문 신청 또는 홍콩 병원관리국 공식 앱인 HA Go(Medical Fee Assistance 메뉴)를 통한 온라인 신청.

-승인 및 적용: 승인 시 전액 또는 일부 감면 증명서(Certificate)가 발급되며, 유효기간(최대 18개월) 동안 공공병원 이용 시 진료비가 자동 면제/감면 처리.
종합사회보장원조(CSSA) 수급자나 75세 이상 노령생활지원금(OALA) 수급자는 별도 심사 없이 신분증 확인으로 자동 적용된다. 또한 긴급 환자의 경우 우선 진료 후 3개월 이내 서류를 보완하는 '조건부 감면' 제도로 치료의 연속성을 보장한다.

Eligibility and Application Methods
Hong Kong's public healthcare system has reached a major turning point. The Hong Kong Hospital Authority has extensively reformed the fee structure for public hospitals with the goal of ensuring sustainable medical finances and preventing the abuse of emergency rooms.
The core of this reform is not simply a price hike. It is the strengthening of "selective public service," which concentrates resources on genuine emergency patients while lowering the barrier to entry for the economically vulnerable.
The system has transitioned from a single-price system to a differential pricing system based on the level of urgency.
Basic Emergency Room (A&E) Fee: Increased from the previous 180 HKD to 400 HKD.
Full Exemption for Severe Patients: Patients classified as Category 1 (Critical) or Category 2 (Emergency) under the Triage classification system will receive free treatment with a full exemption of the 400 HKD fee. General Outpatient and Examination Fees: Fees for general family medicine outpatient treatments and injections have been adjusted from 19 HKD to 50 HKD.
A co-payment system ranging from 50 to 500 HKD has been introduced for high-precision imaging (CT and MRI) and pathology examinations.
Annual Co-payment Cap: To prevent a surge in medical expenses for patients, an annual co-payment cap of 10,000 HKD has been established.
Expansion of Eligibility for Low-Income Groups
To prevent medical exclusion among low-income groups due to rising medical costs, the Hong Kong government has significantly relaxed the Means Test criteria.
Eligibility requirements have been eased to below 150% of the median income for single-person households and below 100% for households with two or more people. Additionally, asset criteria have been raised to the level required for public housing applications, thereby expanding the scope of coverage. As a result, the number of beneficiaries for the regular medical fee waiver has expanded significantly from a mere 14,000 in the past to over 1.4 million, earning praise for substantially improving practical access to medical care for vulnerable groups.
Medical Fee Waiver Application Guide
To receive the waiver benefit, verification of eligibility and a document review process are required.
Application Channels: Apply in person at the Medical Social Service Unit (MSSU) within each public hospital or apply online via HA Go (Medical Fee Assistance menu), the official app of the Hong Kong Hospital Authority.
Required Documents:
Copies of Hong Kong Identity Cards (HKID) for the patient and household members
Proof of household income for the past 6 months (pay slips, bank transaction records, MPF records, etc.)
Proof of assets (bank balances, stocks/insurance assets, proof of real estate ownership, etc.)
- Application Procedure:
Preliminary Assessment: Check eligibility for the waiver in advance using the 'Means Test Calculator' within the HA Go app.
- Document Submission and Review: An assessment of combined household income and assets is conducted through an interview with the assigned Medical Social Worker (MSW).
- Approval and Application: Upon approval, a Certificate of Full or Partial Waiver is issued. During the validity period (up to 18 months), medical fees are automatically waived or reduced when using public hospitals.
Recipients of Comprehensive Social Security Assistance (CSSA) or Old Age Living Assistance (OALA) for those aged 75 and older are automatically eligible upon identity verification without a separate review. Additionally, for emergency patients, the continuity of treatment is ensured through a "Conditional Waiver" system, requiring supplementary documentation within three months of initial treatment.
